On or about September 19, 2014, employees walking inside the home while other employees are decking and installing facia on the roof were not wearing hard hats, exposing themselves to head injuries.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. On or about September 19, 2014, back side of home, employees were using a Louisville 28-foot extension ladder to access and exit from the roof.  The ladder was not extended 3 feet past the landing, exposing the employees to fall hazards.    b. On or about September 19, 2014, front of the home, employees were using a 20-foot extension ladder to access and exit from the roof.  The ladder was not extended 3 feet past the landing, exposing the employees to fall hazards.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:    a. On or about September 19, 2014, front of the home, employees were using a blue 20-foot extension ladder to access and exit from the roof area.  The ladder was not extended 3 feet and had a large split or damaged side rail, exposing the employees to fall hazards.  The employer did not withdraw the ladder from service or mark it as defective nor tagged it with "DO NOT USE".

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. On or about September 19, 2014, employees decking a 7:12 pitch roof were not protected from falling 9 feet 4-inches by the use of a guardrail system, safety net system or a personal fall arrest system.    b. On or about September 19, 2014, the employee installing facia on a 7:12 pitch roof was not protected from falling 9 feet 4-inches by the use of a guardrail system, safety net system or a personal fall arrest system.    The Wildkatz, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 944914, citation number 01, item number 001 and was affirmed as a final order on December 19, 2013, with respect to a workplace located at 424 Wingspan Drive, Bunnell, FL.
