29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork while the platform was occupied: (a) 29 starboard Way, Mt. Laurel, NJ: On nor about 9/25/2014, an employee of AVA Construction was using a plywood platform raised on the forks of a Pettibone rough terrain fork lift to conduct trim work on the second floor windows of a residential structure under construction. The platform was not attached in any way to the forks.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection: (a) 29 starboard Way, Mt. Laurel, NJ: On nor about 9/25/2014, an employee of AVA Construction was using a plywood platform raised on the forks of a Pettibone rough terrain fork lift to conduct trim work on the second floor windows of a residential structure under construction. The employee was not using any form of fall protection and was exposed to a fall hazard of approximately 18 feet to the ground below.
