29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     (a)          JBS Five Rivers Cattle Feeding LLC at 28625 US Highway 34, Kersey, CO 80644: On and before 9/18/14, the employer did not protect employees from hazards created by operating a flaker unit with an opening to an unused portion of the rotating belt that was unguarded. This condition exposed employees to amputation hazards.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.22(a)(2): The floor of every workroom shall be maintained in a clean and, so far as possible, a dry condition. Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable.   (a)          JBS Five Rivers Cattle Feeding LLC at 28625 US Highway 34, Kersey, CO 80644:  On and before 9/18/14, the employer did not ensure that the floor of every workroom was maintained in a clean, dry condition. The boiler room had standing water and the condition exposed employees to a slip hazard. (a)          JBS Five Rivers Cattle Feeding LLC at 28625 US Highway 34, Kersey, CO 80644:  On and before 1/23/15, the employer did not ensure that the lockout or tagout devices were affixed to each energy isolating device by authorized employees. This condition potentially exposed employees and employees of contractors to hazardous energy sources.
