29 CFR 1928.57(a)(6):  At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he/she was or would be involved:  (a) JBS Five Rivers Cattle Feeding, LLC @ 38002 County Road N, Yuma Colorado 80759.  On or before January 26, 2016, as part of the injury/accident prevention responsibilities under its safety program, the employer did not ensure that each employee was instructed in the safe operation and servicing of the Flaker Machines in the Mill Dept.  Such instruction did not include at least the following safe operating work practices:    (1)  Stopping the motors, disconnecting power sources, and waiting for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment.  Where the machine must be running to be properly serviced or maintained, the employer shall instruct employees as to all steps and procedures which are necessary to safely service or maintain the equipment;  (2)  Locking out all associated hazardous energies such as, but not limited to, the kinetic energy generated by the rollers of the Flaker Machines as they continued to turn, before performing maintenance or service on the machine or equipment.   An employee experienced an amputation of three fingers when attempting to unclog a Flaker Machine roller.  Condition exposed employees to amputation hazards when operating, adjusting, cleaning or unclogging the Flaker Machines.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an amputation hazard.    (a) JBS Five Rivers Cattle Feeding, LLC @ 38002 County Road N, Yuma Colorado 80759.  On or before January 26, 2016, employees were exposed to an amputation hazard when performing maintenance of flaker machines in that all stored energy in the flakers had not been relieved and blocked. This condition exposed employees to an amputation hazard:    One feasible method of abatement was to ensure that all stored energy had been relieved and blocked.
