Packers Avenue Building/North West Corner - Welding Area - On or about October 10, 2014, the employer did not ensure that employees required to control the flow of raw materials and finished products to and from the welding lines were provided with appropriate eye protection. Employees were thereby exposed to hazards of ultraviolet radiation.

Where cutting or welding is done near walls, partitions, ceiling or roof of combustible construction, fire-resistant shields or guards shall be provided to prevent ignition.  a. 4400 S. packers Avenue Building/North West Wall/Welding Area/Sections - On or about October 10, 2014, employees were required to conduct welding (MIG and SPOT) operations and combustible materials/products were being used as screens and curtains. Employees were thereby exposed to the physical hazards of ultraviolet radiation. The conditions were the following;  1.  At three (3) welding areas/sections cardboard materials/boxes parts were being used as walls or protective partitions.

29 CFR 1910.252(c)(1)(ii): Screens. When welding must be performed in a space entirely screened on all sides, the screens shall be so arranged that no serious restriction of ventilation exists. It is desirable to have the screens so mounted that they are about 2 feet (0.61 m) above the floor unless the work is performed at so low a level that the screen must be extended nearer to the floor to protect nearby workers from the glare of welding.  a. 4400 S. Packers Avenue Building/North West corner/Welding Area - On or about October 10, 2014, employees were required to perform welding (MIG and SPOT) operations in spaces that were not entirely screened on all sides. Employees were thereby exposed to the physical hazards of ultraviolet radiation.   1. There were seven (7) welding spaces/areas in the main center aisle and around the exit door # 9 that were not totally screened on all sides.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.    a. Mechanic Shop Area - On or about January 21, 2015, the employer had not provided information and training to all employees who were required to conduct machine repair/maintenance operations while using hazardous chemicals such as oils and lubricants. Employees were thereby exposed to the chemical hazards of aliphatic hydrocarbons.

Packers Avenue Building/Welding Area/Near the Exit Door # 9 - On or about October 10, 2014, the employer had not ensure that -employee owned equipment - for welders was adequate and properly maintained. Employees were potentially exposed to the physical hazards of ultraviolet radiation. The employee owned equipment was the following;  1. The welder's helmet - Jackson KC1513-400 - had its interior glass held in place by green adhesive tape around its perimeter.

Women's Washrooms - On or about September 23, 2014, all toilet facilities at the company were not adequately provided with sanitary absorbent paper. Employees were thereby potentially exposed to health related illnesses. The conditions found were the following;  1. At the 4400 S. Packers Avenue Building/ North West Corner/Women's Washroom - there were three (3) stalls in the employee's washroom which were not provided with sanitary absorbent paper.
