29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:        a.   On or about 09/23/2014, Stop and Shop Supermarket - 5204 Hannas Rest- receiving ramp area, St. Croix, VI:  Employee were exposed to fall hazards while working over an open side floor 5.5 feet above the ground level, without a conventional fall protection system in place.      NOTE:  Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).

29 CFR 1910.23(d)(1)(iv): On stairways more than 44 inches wide but less than 88 inches wide were not equipped with one standard handrail on each enclosed side and one standard stair railing on each side:    a.    On or about 09/23/2014, Stop and Shop Supermarket - 5204 Hannas Rest- loading stairway area, St. Croix, VI :  Employees were exposed to fall hazards while working over a stairway that was not equipped with one standard stair railing on each side. 29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    On or about 09/23/2014, Stop and Shop Supermarket - 5204 Hannas Rest- Walk-in Meat freezer area, St. Croix, VI: :  The freezer inside knob release part of the door safety latch mechanism was not working, thus in case that the freezer door was inadvertently closed employees traveling in and out from the walk-in freezer are exposed to being locked inside.    Note:  Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).

29 CFR 1910.178(f)(2):   The storage and handling of liquefied petroleum gas fuel was not in accordance with NFPA No. 58-1969, Storage and Handling of Liquefied Petroleum Gases:      Section 5.1:  General:  "Containers when stored inside shall not located near exits, stairways or in areas normally used or intended for the safe exit of people."    Instance #1:   On or about 09/23/2014, Stop and Shop Supermarket - 5204 Hannas Rest- receiving area, St. Croix, VI:   LPG tanks were stored next to steel rack which is near to receiving warehouse door & supermarket hallways exposing employees to fire hazards while working at the area.      Section 5.2(a): Storage within Buildings Frequented by the Public:  The total quantity on display and in storage shall not exceed 200 pounds LP-Gas"       Note for instance #1:  Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

The employer did not certify that each operator had been trained and evaluated.
