29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:                                                                                                              On or about October 1, 2014, periodic inspections were not conducted of lockout/tagout procedures necessary to control the hazardous energy of machines such as, but not limited to, the Omax Water Jet, Plasma Cutter, Warner Swasey Lathe #5, Edwards Manufacturing Co. Ironworker and Hydra Tool Co. Inc. 10 Ton Press. The lockout/tagout procedures for this equipment failed to meet the requirements of the standard, exposing employees to injury during servicing and maintenance.
