29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Vehicle maintenance garage - Energy isolating devices used to control gravitational energy and the potential conversion of gravitational energy to kinetic energy were not located or used in a manner to isolate the equipment from the energy source when supporting a raised 1999 Club Car Carryall II. Wheel chocks were not placed on the wheels remaining on the floor and jack stands were not placed in a proper location to support the front of the vehicle, thereby exposing an employee to crushing hazards. This condition was observed on or about September 29, 2014.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Vehicle Maintenance Garage - The employer does not have a suitable facility for quick drenching or flushing of eyes within the work area for immediate emergency use thus exposing the employee to optical injuries. The employee is exposed to injurious corrosive material in that the employee is required to fill batteries with water on the electric utility vehicles. This condition was observed on or about October 3, 2014.
