29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   b) At the jobsite: 1607 Ogden Street - Employees on a pump jack scaffold were not protected from falling by the use of personal fall arrest systems or guardrail systems. The employees were exposed to fall hazards of approximately twenty-five (25) feet above ground level. On or about 10/01/2014.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a) At the jobsite: 1607 Ogden Street - The employer did not provide a safety training program for each employee working from a pump jack scaffold. The employees did not have the knowledge to recognize the hazards and they did not understand the procedures to minimize the hazards associated with pump jack scaffolds. The ladder was six feet in length and the scaffold was located approx. 14 feet above the ground. On or about 10/01/14.
