29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:  a) Southern Wood Works of Gray, LLC.- Gray, GA-Blocks of wood and wood debris were accumulating in the production area and aisle ways creating a tripping condition for employees walking and operating the forklift, on or about October 1, 2014.  b)Southern Wood Works of Gray, LLC.- Gray, GA-A 240 volt electrical cord that supplies power to the Pendue Pointer machine created a tripping hazard for employees working between the Pendue Pointer and pointer #2, on or about October 1, 2014.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Noise monitoring to determine the levels of noise employees were exposed to had not been conducted, on or about October 1, 2014. 29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  a) Southern Wood Works of Gray, LLC.- Gray, GA - In the Sorting Department, an employee sorting wood was exposed to noise at a time-weighted average (TWA) of  98.7 dBA.  An audiometric testing program was not maintained for the employees.  The sample was obtained from a 420 minute sampling period, which represents the employee's eight hour work day, on or about October 28, 2014.  b) Southern Wood Works of Gray, LLC.- Gray, GA - In the Sawing Department, an employee cutting wood was exposed to noise at a time-weighted average (TWA) of 96.7 dBA.  An audiometric testing program was not maintained for the employees.  The sample was obtained from a 394 minute sampling period, which represents the employee's eight hour work day on or about October 28, 2014.  c) Southern Wood Works of Gray, LLC.- Gray, GA - In the Pointer Department, an employee cutting and stacking wood pointers was exposed to noise at a time-weighted average (TWA) of 95.3 dBA.  An audiometric testing program was not maintained for the employees.  The sample was obtained from a 424 minute sampling period, which represents the employee's eight hour work day, on or about October 28, 2014.
