29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Fabrication Area - On October 2, 2014, La Marche Mfg. Co. did not ensure the point of operation was guarded on the Hydra Tool Company press brake (s/n - S17002).      In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock being cut to give maximum protection for the operation being performed.       a) Skid Building Area - On October 2, 2014, La Marche Mfg. Co. did not ensure the lower exposed portion of the saw blade on the Delta Rockwell radial saw was guarded by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock throughout the cut when the saw was operated by employees.     No abatement certification or documentation is required for this item.
