29 CFR 1910.303(b)(1)(i): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the consideration of suitability for installation and use in conformity with the provisions of this subpart as evidenced by listing or labeling for that identified purpose: On or about October 2, 2014, in the employee break room, two relocatable power taps (RPT) were interconnected or "daisy chained" to provide more outlets. A TV, cable box, Ice Beverage Dispenser, Ice Tea machine, microwave, toaster, and coffee machine were energized by these RPT's.

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used: On or about October 2, 2014, in the employee break room, the cord to the surge protector was run through a hole in the countertop and plugged into an outlet. The surge protector supplied power to appliances.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about October 2, 2014, at this facility, at the rear loading docks, employees used the dock leveler on a daily basis to raise the ramps up and down for the oncoming trailers and strain relief was not provided on the leveler connections such as but not limited to: (a) The bottom cable connected to the dock leveler between 3 & 4; (b) The bottom cable connected to the dock leveler between 2 & 3 had been taped. Change Standard to: 29 CFR 1910.305(b)(1)

29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On October 2, 2014, at the southeast corner of the warehouse, by dock 7, the employee's radio was connected to the outlet and it did not have a face plate cover. The employees were exposed to contact with live conductors.
