OSH ACT of 1970 Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: A.) Fansteel Inc. dba American Sintered Technologies, Powder Room , MAC dust collector - On or about 10/2/14, employees were potentially exposed to fire and explosion hazards from a dust collector located inside. Dust collectors including but not limited to, the MAC standard model 2M2F8 were located inside the building and were not equipped with any devices to mitigate a potential dust explosion. The dust collector collected combustible carbon, graphite and metal dust. B.) Fansteel Inc. dba American Sintered Technologies, Powder Room , MAC dust collector - On or about 10/2/14, employees were potentially exposed to fire and explosion hazards from dust collectors, MAC standard model 2M2F8, that were not equipped with devices and systems to prevent the propagation of explosions and associated flame fronts from the dust collector through the associated system duct work that recirculates the air. MAC dust collector was not equipped with approved devices to prevent the propagation of a dust explosion and associated flame fronts throughout the dust collection system. The dust collector collected carbon, graphite, and metal dust.

Fansteel Inc. dba American Sintered Technologies, Powder Room - On or about 10/2/14, proper housekeeping was not maintained in the Powder Room. Combustible dust had accumulated throughout the Powder Room on the walls, duct work, and beams.. In addition, the dust had accumulated on electrical system components throughout the room. These electrical system components included but were not limited to electrical boxes, disconnects, conduit, and outlets. The combustible dust included but was not limited to graphite dust which contained some metal. The condition exposed personnel to a fire and explosion hazard.
