29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill: (a) Jobsite: On or about October 2, 2014, the employer did not retrain employees who were working on the roof to ensure that the employees had the required skill and knowledge to minimize fall hazards while working on the roof.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Jobsite: On or about October 2, 2014, six employees were working near the edge of the roof and they were not protected from falling, nor was there a warning line set up (where the employees were working) nor was there a safety monitor being used at the time of the inspection. The dimensions on the roof were 125 feet by 125 feet. The fall hazard to the employees was about 15 feet to pavement. The Kalkreuth Roofing & Sheet Metal Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 982076, citation number 2, item number 1 and was affirmed as a final order on 7/29/2014, with respect to a workplace located at 4209 State Route 44304-215-7029, Rootstown, OH 44272.
