Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. (a) Willow Lane: On or about June 6, 2017, and times prior, employees performing roofing work on a flat roof, 60ft. by 168ft., did not utilize a safety monitoring system along with the warning line system, to protect employees from a 30 foot fall.

The plywood box attachment was open across the front, attached to the backrest of the forklift and was separating on the bottom and sides due to overloading.
