29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).   On or about October 1, 2014, on the east side of the residential house, employees performing masonry activities from the scaffold platform that was not fully planked were exposed to fall hazards of approximately 13 feet to the ground.  The Margarito Ramirez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 920153, citation number 1, item number 6a and was affirmed as a final order on September 20, 2013, with a final abatement date of September 25, 2013, with respect to a workplace located at 1309 Twisting Meadows Drive, Haslet, Texas 76052.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access.  On or about October 1, 2014, on the east side of the residential house, employees performing masonry activities were not provided a ladder or safe access and egress to working levels on the scaffold system exposing the employees to fall hazards of approximately 13 feet to the ground below.      Margarito Ramirez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 920153, citation number 1, item number 6b and was affirmed as a final order on September 20, 2013, with a final abatement date of September 25, 2013, with respect to a workplace located at 1309 Twisting Meadows Drive, Haslet, Texas 76052.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.   On or about October 1, 2014, on the east side of the residential house, guardrails were not installed along all open sides, and end frames on the tubular welded frame scaffold where employees were performing masonry activities, exposing them to fall hazards of approximately 13 feet to the ground below.    The Margarito Ramirez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 920153, citation number 1, item number 6d and was affirmed as a final order on September 20, 2013, with a final abatement date of September 25, 2013, with respect to a workplace located at 1309 Twisting Meadows Drive, Haslet, Texas 76052.
