29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about October 01, 2014, employees were using an aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the roof.  The employees were exposed to a fall hazard.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.   On or about October 01, 2014, the employer did not ensure employees engaged in roofing activities were trained to recognize and avoid hazards related to ladders and the procedures to be followed in order to minimize or avoid those hazards.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  On or about October 01, 2014,  employees engaged in residential roofing activities installing roofing material were not provided fall protection, exposing them to an unprotected fall hazard of 11 feet to the ground below.    Luis Lera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 920205, Citation Number 1, Item number 1 and was affirmed as a final order on November 01, 2013, with a final abatement date of October 21, 2014, with respect to a workplace located at 3301 Glen Crest Lane, Denton, TX 76106.
