29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about June 12, 2024, employees roofing a house were exposed to falls in that the following ladders did not have their side rails at least 3 feet above the roof line: (1) A Werner aluminum extension ladder placed at the rear of the house extended to the first level roofline approximately 10 feet above with its side rails approximately 2.5 feet above the roofline. (2) A single section of an aluminum extension ladder was placed at the rear of the house on top of the first-level roof and leaning up against the second level roofline with its side rails approximately 1.5 feet above the roofline. (3) A Werner aluminum extension ladder placed on the side of the house extended to the second level roofline approximately twenty-one (21) feet above with its side rails approximately 2.5 feet above the roofline. (4) A Werner aluminum extension ladder placed on the side of the house near the front extended to the first level roofline approximately 10 feet above with its side rails approximately 2 feet above the roofline.
