29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) The employer failed to provide documentation and certification for abatement of citation 1 item 1 in which the employer did not develop an accident prevention program, that that provided frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer. Condition noted on or about 10/02/2014.  b) The employer failed to provide documentation and certification for abatement of citation 1 item 2 in which stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches or more. Condition observed on or about 10/2/2014.  c) The employer failed to provide documentation and certification for abatement of citation 2 item 1 in which each employees were not protected by guardrail systems, safety net systems, or personal fall arrest system. Condition observed on or about 10/02/2014.  d) The employer failed to provide documentation and certification for abatement of citation 2 item 2 in which the employer did not provide a training program for each employee exposed to fall hazards. Condition noted on or about 10/02/2014.

29 CFR 1926.20(b)(2): The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer.   a) 10 Grant Wood Way, Evesham, NJ:  On or about 10/02/2014 three 4x4 General Contractors Inc. employees were  conducting framing activities on a new construction multi story residential structure.  The employees were exposed to hazards related to a lack of fall protection as well as a failure to provide proper access to elevated levels. The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches or more:    a) 10 Grant Wood Way, Evesham, NJ:  On or about 10/02/2014 three 4x4 General Contractors Inc. employees were  conducting framing activities on a new construction multi story residential structure.  The employees were working on the second story decking and were not provided with proper access (stairways or ladders)to the level.  Employees were observed to climb down using the structure's framed walls, exposing them to a fall hazard of greater than 8 feet.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   a) 10 Grant Wood Way, Evesham, NJ:  On or about 10/02/2014 three 4x4 General Contractors Inc. employees were conducting framing activities on a new construction multi story residential structure.  The employees were decking the second story and walking the exterior top plate.  They were not using any form of fall protection, thus exposing them to a fall hazard of greater than 14 feet.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.    4x4 General Contractors Inc.,  was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 989466, citation number 02, item number 01a and was affirmed as a final order on 09/08/2014, with respect to a workplace located at 219 West George Street, Philadelphia, PA 19123.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  a) 10 Grant Wood Way, Evesham, NJ:  On or about 10/02/2014 three 4x4 General Contractors Inc. employees were conducting framing activities on a new construction multi story residential structure.  The employees were decking the second story and walking the exterior top plate.  They were not using any form of fall protection, thus exposing them to a fall hazard of greater than 14 feet.   The employer had not trained each employee who was exposed to fall hazards to recognize the hazards and the procedures to be followed to minimize those hazards.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
