29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   a.    Throughout facility - On or about October 07, 2014, employees (full-time / temporary) were required to operate / maintain / repair equipment without proper lockout / tagout procedures. The employer did not establish an energy control program for machine operators and/or maintenance employees  performing motor changes on rewind machines and other operating equipment, thereby exposing employees to the hazard of unexpected energization of equipment.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:   a.    Throughout production area - On or about October 07, 2014, employees were required to clean and / or change / set dies on various machinery (Spoolers / Re-winders / Wire manufacturing equipment) on a daily basis. The employer did not have written procedures for the control of hazardous energy where employees were required to clean and / or change / set dies. Employees were thereby exposed to the hazards associated with the unexpected reenergization of equipment.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.  a.    Throughout production area - On or about October 07, 2014, employees were required to clean and/or change / set dies on various machinery on a daily basis. The employer did not ensure that employees affixed a lockout / tagout device prior to cleaning and / or changing / setting dies. Employees were thereby exposed to the hazards associated with the unexpected reenergization of equipment.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.   a.    Production Area - On or about October 07, 2014, employees (full-time / temporary) were required to operate the Reelex model DHS-750 Spooler, serial number 02832 on a daily basis. The rotating parts of the machines were not properly guarded, thereby exposing employees to the hazards associated with rotating parts.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.   a.    Production area - On or about October 07, 2014, employees were required to operate various machinery powered by conductors not properly protected from abrasion in accordance with OSHA standards, thereby exposing employees to the hazards associated with electricity (electrocution, fire, and arc flash).

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  [63 FR 33450, June 18, 1998]    a.     On or about October 07, 2014, in the production area, employees required to add water to the forklift battery were potentially exposed to sulfuric acid and not provided with suitable facilities for quick drenching or flushing of eyes and body. Access to the eye wash station was blocked  by a pallet at the time of inspection.
