29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:  (a) Jobsite, Walnut & Eden Park Boulevard, McKeesport, PA:  On or about 10/01/14 an employee working in an aerial lift approximately 11-15 feet above the ground was not wearing a harness and lanyard tied off to the boom or basket.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  (a) Jobsite, Walnut Street & Eden Park Boulevard, McKeesport, PA:  On or about 10/01/14 employees engaged in steel erection activities more than 15 feet above the ground were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, positioning devices or fall restraints.
