On or about October 15, 2014, at the paint mix and storage trailer, there was no self-closing door for the opening to the paint building; there was no raised sill, ramp, or open-grated trench where the floor storage area was the same level throughout the trailer.  Flammable liquids were stored such as coatings, primers, and thinners in excess of 120 gallons, which exposes employees to fire and explosion hazards.
