OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to explosion hazards and sharp particles flying in the air: a) Production Area - Employees were exposed to flying plastic projectile hazards due to possible mechanical failure of the plastic polyvinyl chloride (PVC) piping system used to transport compressed air, exposing employees to serious physical hazards which could result in lacerations to the body and damage to the eyes resulting in hospitalization.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication training did not include an explanation of the Global Harmonization System (GHS) with respect to the labeling, pictograms, and safety data sheets, including the order of information and how employees could obtain and use the appropriate hazard information: a) Throughout the Facility: The employer did not provide training to employees on the new hazard communication standard Global Harmonized System (GHS) that took effective December 1, 2013. Employees were not familiar with the new hazard communication standard. Employees were exposed to hazardous chemicals in the workplace, such as but limited to, propane, acetone, alcohols, and spray inks.
