29 CFR  1910.95(c)(1):The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) Production Department - On or about February 11, 2022, employees were exposed to continuous noise at 185% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or equivalent sound levels of 94 dBA during 395 minutes sampling period. The exposure calculation included a zero increment of the 85 minutes not sampled. The employer did not implement a continuing and effective hearing conservation program for employees exposed to potential hearing loss while operating press - plastic injection molding machine #1 (model # kx360).  b) Production Department - On or about February 11, 2022, employees were exposed to continuous noise at 80% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or equivalent sound levels of 88 dBA during 395 minutes sampling period. The exposure calculation included a zero increment of the 85 minutes not sampled. The employer did not implement a continuing and effective hearing conservation program for employees exposed to potential hearing loss while operating press - plastic injection molding machines # 14, 17 and 18.    c) Production Department - On or about February 11, 2022, employees were exposed to continuous noise at 70% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or equivalent sound levels of 87 dBA during 375 minutes sampling period. The exposure calculation included a zero increment of the 85 minutes not sampled. The employer did not implement a continuing and effective hearing conservation program for employees exposed to potential hearing loss while operating screw machine.  d) Production Department - On or about February 11, 2022, employees were exposed to continuous noise at 60% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or equivalent sound levels of 86 dBA during 375 minutes sampling period. The exposure calculation included a zero increment of the 85 minutes not sampled. The employer did not implement a continuing and effective hearing conservation program for employees exposed to potential hearing loss while operating drill machine.

29 CFR  1910.133(a)(3): The employer did not ensure that employees wearing prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design or wears eye protection that can be worn over the prescription lenses.   a) Production Department - On or about February 2, 2022, employees were exposed to eye hazards in that the employer failed to ensure that each affected employees wears eye protection that incorporates the prescription in its design or wears eye protection that can be worn over the prescription lenses when employees were operating press/plastic injection molding machines, screw machine and drill machine.
