29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  (a) Employees were exposed to hazardous energy sources as there were no machine specific lockout tagout procedures for equipment and machinery that was hardwired in, for equipment such as, the Main Unitizer.

29 CFR 1910.179(j)(2): The following items were not be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections:  (a) 7 Color Hy-Corr Area:  Employees were exposed to overhead hazards as the Gorbel 2 ton capacity crane did not have frequent inspections which are at daily to monthly intervals. The inspections are to include inspecting for defects of the crane, hooks that have deformations or cracks, and hoist chains for excessive wear.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) Maintenance Shop:  Employees were exposed to rotating parts from the Central Machinery Milling/Drilling Machine which did not have a guard.  (b) Maintenance Shop:  Employees were exposed to rotating parts as the Jet Turret Milling Machine was not guarded.  (c) Maintenance Shop:  Employees were exposed to amputation hazards as the Jet 9 inch Horizontal Band Saw did not have the unused portion of the blade guarded.

Metal duplex outlet boxes with knockouts are not listed or labeled to be used as a pendant, they are listed or labeled to be attached to a wall or a piece of equipment.
