29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: a) At Bennett Packaging: The employer do not provided affected employee with an annual hearing test evaluation. 1) An employee working in the automantan line was exposed to continuous noise levels at 63% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 86.71 dBA. The sampling was performed for 487 minutes during one shift on October 21, 2014. 2) An employee working in the transline line (infeeder) was exposed to continuous noise levels at 120.7% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 91.36 dBA. The sampling was performed for 465 minutes during one shift on October 21, 2014. 3) An employee working in the transline line (outfeeder) was exposed to continuous noise levels at 112.7% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 90.86 dBA. The sampling was performed for 464 minutes during one shift on October 21, 2014. 4) An employee working in the transline line (outfeeder) was exposed to continuous noise levels at 89% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 89.16 dBA. The sampling was performed for 462 minutes during one shift on October 21, 2014. 5) An employee working in the transline line (infeeder) was exposed to continuous noise levels at 116.5% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 91.10 dBA. The sampling was performed for 460 minutes during one shift on October 21, 2014. 6) An employee working in the Hycorr line (infeeder) was exposed to continuous noise levels at 120% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 91.32 dBA. The sampling was performed for 468 minutes during one shift on October 21, 2014. 7) An employee working in the Hycorr line (feeder) was exposed to continuous noise levels at 167.3% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 93.71 dBA. The sampling was performed for 463 minutes during one shift on October 21, 2014. 8) An employee working in the Young Shin (flatbed feeder) line was exposed to continuous noise levels at 67.7% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 87.19 dBA. The sampling was performed for 456 minutes during one shift on October 21, 2014. 9) An employee working in the Jumbo stacker line (outfeeder) was exposed to continuous noise levels at 56.2% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 85.84 dBA. The sampling was performed for 455 minutes during one shift on October 21, 2014. 10) An employee working in the Baller Bloapco line was exposed to continuous noise levels at 95.5% of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent sound level of approximately 89.67 dBA. The sampling was performed for 447 minutes during one shift on October 21, 2014.
