Nevada Avenue, Colorado Springs, CO 80907, with no fall protection in place.                  2. On October 2, 2014, Employees of 5 Star Roofing, LLC., were exposed to fall hazards in excess of 12 feet while engaged in roofing activities on the roof of a residential building, located at 1923 N. Nevada Avenue, Colorado Springs, CO 80907, with no fall protection in place.
