Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: At a new residential construction, East side, on or about October 8, 2014, employees were exposed to an electrical hazard while using a 120V wall electrical box that was not mounted on the wall, yet supplying power to the Skilsaw circular saw (Model: 5480, SN: 40308) used to cut metal roofing panels.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At a new residential construction, East side, on or about October 8, 2014, employees were exposed to a fall hazard greater than 6 feet in height to the unpaved ground level below while working from a roof with a pitch of 8:12 without fall protection; installing metal roofing panels on a new residential home.
