29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  Employees operating the company forklifts were exposed to struck-by hazards. The employees were exposed in that class room training and a demonstration exercise for the employees to complete was not presented to minimize the hazards of operating the forklifts.

29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects:  In the Shop area an employee engaged in welding with the use of a Miller welder (SN-JJ430717) was exposed to electrical shock. The employee was exposed in that the terminals on the welder did not have safety covers to prevent accidental contact by metal objects or personnel.
