29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: On or about October 7, 2014 at the Graham Packaging Facility- employees in the main production building were exposed to a struck by hazard from possible failure of hoist chains because monthly inspections for excessive wear, twists, or distorted links were not accomplished or documented.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about October 7, 2014 at the Graham Packaging Facility- employees were exposed to serious injuries, in that the Electrical Panel located in the NE Corner of the warehouse had knock out openings that had not been effectively closed. Abatement verification for this item is not required.
