29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about 10/10/2014, at the above addressed location, employees working in an excavation greater than 4-feet deep did not have a ladder for access exposing them to a fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about 10/10/2014, at the above addressed location, employees were exposed to a fall hazard while using a ladder in an excavation that did not extend at least 3-feet above the landing surface.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59):  On or about 10/10/2014, at the above addressed jobsite, the employer did not have a written hazard communication program for employees working in an excavation and exposed to hazardous chemicals such as, but not limited to Portland cement.
