OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in-between hazards: On 9/13/2016, at the above addressed work site, employees were exposed to a caught-in-between hazard during the hoisting operation of sheet piles which the machine operator and employees rigging the equipment failed to effectively communicate with each other during the removal of the pelican hooks during the hoisting operation. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
