On or about October 9, 2014, employees working on the roof of a residential home without fall protection were exposed to an unprotected fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about October 9, 2014, employees were accessing a residential roof with a portable ladder that did not extend at least 3 feet above the landing exposing employees to the hazard of falling to the ground below.

On or about October 9, 2014, employees performing roofing activities on a residential roof were using an A-Frame ladder in the closed position and leaned up against the 8/12 pitch roof exposing the employee to a fall hazard of approximately 20 feet to the ground below.
