29 CFR 1910.119(n): The employer did not implement an emergency plan for the entire plant in accordance with the provisions of 29 CFR 1910.38: The employer did not ensure that the emergency action plan was implemented in that on or about October 6, 2014 a fire occurred at Silo 8020 and employees such as but not limited to the plant manager, EHS Manager, Maintenance Manager, maintenance personnel and outside contractor did not evacuate until an explosion occurred.
