29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Apartment Building 29, 30, 31, Construction Site: On or about October 10, 2014, and at times prior, employees were exposed to a fall of approximately eighteen feet to the ground below while installing composite asphalt shingles on a 5:12 pitch steep slope roof. (b) Apartment Building 32,33,34, Construction Site: On or about October 10, 2014, and at times prior, employees were exposed to a fall of approximately sixteen feet to the ground below while installing composite asphalt shingles on a 5:12 pitch steep slope roof. (c) Apartment Building 35,36,37,38, Construction Site: On or about October 10, 2014, and at times prior, employees were exposed to a fall of approximately eighteen feet to the ground below while installing composite asphalt shingles on a 5:12 pitch steep slope roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Apartment Building 35,36,37,38, Construction Site: On or about October 10, 2014, and at times prior, employees were exposed to a fall of approximately eighteen feet to the ground below while accessing the roof on a ladder that extended approximately eighteen inches above the access level. (b) Apartment Building 29,30,31, Construction Site: On or about October 10, 2014, and at times prior, employees were exposed to a fall of approximately nine feet to the ground below while accessing the roof on a ladder that extended approximately twelve inches above the access level.
