29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems: On or about October 14, 2014 and at times prior thereto, employees were working on the roof of a residential building. The employees were wearing harnesses but weren't attached to an anchorage point. Employees were exposed to fall hazards of greater than 6 feet to the ground below.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about October 14, 2014 and at times prior thereto, the employer failed to instruct his employees in the recognition of unsafe conditions or how to eliminate any hazards. Employees were using harnesses but weren't attached to an anchor point which exposed them to fall hazards.
