Employees were exposed to lead, toxic chemicals, and flying projectiles while conducting maintenance on firearms. The employer had not certified that a personal protective equipment hazard assessment had been performed. Employees were exposed to the following, but not limited to, hazards: 1. Exposure to lead while test firing guns; 2. Exposure to lead when cleaning the trap located at the bottom of the snail; 3. Exposure to flying debris while polishing or grinding using the bench grinder.

The employer required the employees who worked in the gun firing room to wear a 3M half face respirator with OV/P100 cartridges. Employees are required to fire guns into a "snail" test fire tube, and clean a waste trap at the bottom of the apparatus, both activities potentially expose employees to lead exposures. The employer had not established or maintained a written respiratory protection program. This condition exposed employees to respiratory hazards associated with exposure to lead.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion. (a) Bass Pro Shops at 7970 Northfield Blvd in Denver, CO: On and preceding, 10/08/14, the employer did not ensure the workrest on the Delta "Shop Master" bench grinder was adjusted to the maximum opening of one-eighth inch in that the measured opening was approximately one-half inch. This condition exposed employees to struck-by and abrasion hazards.

As part of their job duties employees fire guns into a "snail" bullet catching apparatus and then clean the waste trap at the bottom of the apparatus. Both activities can result in employee exposures to lead. Without conducting an initial determination employees were exposed to lead hazards.
