The employer did not instruct each employee in the recognition and avoidance of unsafe conditions such as but not limited to, using adequate protection to protect employees from loose rock or soil falling or rolling from an excavation face and having no adequate protective system(s). Employees were exposed employees to cave-in and crushed by hazards.
