29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a. On or about October 15, 2014, at west side of the roof, three employees were exposed to a fall hazard when ascending and descending from a rooftop on a ladder which was not extended at least 3 feet above the upper landing surface due to their length. The ladder was not secure at its top and a grasping device was not provided.

K&D Roofing & Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315482257, citation number 01, item number 001 and was affirmed as a final order on March 21, 2012, with respect to a workplace located at 1852 & 1837 Blue Bonnet Way, Orange Park, FL 32003.    29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a. On or about October 15, 2014, at the south side of the residence and west side of the roof, four employees using pneumatic nailers to install shingles were not wearing eye protection, exposing themselves to being struck by flying debris or nails.

K&D Roofing & Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315482257, citation number 02, item number 001 and was affirmed as a final order on March 21, 2012, with respect to a workplace located at 1852 & 1837 Blue Bonnet Way, Orange Park, FL 32003.    29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. On or about October 15, 2014, two employees installing shingles on a 6:12 pitch roof were exposed to an 8 ft. 3 in. fall hazard without the use of personal fall arrest system, safety net system or guardrail system.    b. On or about October 15, 2014, an employee installing shingles on a 6:12 pitch roof was exposed to a 12 ft. 3 in. fall hazard without the use of personal fall arrest system, safety net system or guardrail system.
