The ladder was not secure at its top and a grasping device was not provided.

On or about October 15, 2014, an employee installing shingles on a 6:12 pitch roof was exposed to a 12 ft. 3 in. fall hazard without the use of personal fall arrest system, safety net system or guardrail system.
