On or about October 7, 2014, the contract employer did not assure that two contract employees performing work for and under the direction of the host employer relating to the transfer of sulfuric acid at the host employer's Chlor-Alkali Unit at its Point Comfort, TX facility were trained in the work practices necessary to safely perform his/her job.

1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. SPVC Unit, at Formosa Plastics Corporation, Point Comfort, Texas: On or about October 7, 2014, the employer did not record the following work place injuries and illnesses on the OSHA 300 Log for calendar year 2014. On April 22, 2014, a Total Housekeeping Management Employee, sustained fatal injuries when he fell 40 feet to the lower level of the tank.
