29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about October 16, 2014, at the job site located at 424 Augusta Ave in Conway, Arkansas, employees were installing weather barrier wrapping across the front of a new student residential facility. The employees were working from a JLG model 450 AG articulating boom aerial lift and were not wearing body belts with lanyards attached to the boom or basket. These conditions exposed the employees to the hazard of falling approximately 14 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 16, 2014, at the job site located at 424 Augusta Ave in Conway, Arkansas, where employees were framing 2 story student residential facilities, employees erecting ceiling joists and decking the roofs were not using any type of fall protection. These conditions exposed the employees to the hazard of falling from heights of 9 to 22 feet to the ground below in the following instances: a) The first story ceiling area of Building H2 a) The second story roof of Building H1
