29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  Employees performing maintenance tasks on equipment on the steel bins were exposed to fall hazards of approximately 29 feet to the ground.  The ladder opening for the fixed industrial ladder was not protected.

29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons can accidentally walk was not protected by either a a removable standard railing or an attendant:  Employees setting up the tripper to fill grain bins were exposed to fall hazards up to 100 feet.  Employees were not provided fall protection while the grating was removed from the hole before the tripper was placed over the floor hole.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:  Employees working above the boot pit in the east grain elevator were exposed to fall hazards of 21 feet 6 inches to the bottom of the concrete boot pit.  The opening around the bucket elevator leg measured between 5 and 9 inches wide and 3 feet long was not guarded.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:  Employees performing housekeeping and maintenance tasks on the motor level of the west grain elevator were exposed to caught-by hazards in that the 5 inch wide drive shaft fro the head pulley on the bucket elevator leg was not guarded.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Employees working in the gallery of the east grain elevator were exposed to electrical shock, fire and explosion hazards.  A knockout was missing on the 480 volt AC electrical disconnect for the gallery conveyor exposing live electrical parts.
