29 CFR 1910.272(k): Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 1/2 inches:  Employees were exposed to fire and explosions hazards in that the truck dump grates were bent and created gaps, which tools and metal can fall into.  a) Left side = 3 3/4 inches; b) Right side = 3.0 inches.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
