29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard: On and before October 15, 2014, employees were walking, working and welding from the top of the truck trailer flatbeds without fall protection. The flatbeds were 54 inches above the work floor, 8 feet wide and 55 feet long.

29 CFR 1910.26(c)(1): Good safe practices in the use and care of ladder equipment were not employed by the users to get maximum serviceability, safety, and to eliminate unnecessary damage of equipment: On and before October 15, 2014, employees were using a portable metal ladder to ascend and descend the top of the flatbed truck trailer that was 54 inches above the work floor. Good safe practices in the use of portable metal ladders were not implemented in that, the top of the ladder was not three feet above the top edge of the flatbed truck trailer.
