29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) Kubota Manufacturing of America Corporation, Gainesville, GA - An employee working at the frame shop assembly was exposed to noise levels at a TWA of 90.3 dBA. The annual audiogram had not been conducted. The exposure was derived by a sample taken over 504 minutes, on or about 10/22/14. b) Kubota Manufacturing of America Corporation, Gainesville, GA - An employee working at the deck line, station 7 was exposed to a projected noise levels of 91.3 dBA. The annual audiogram had not been conducted. The exposure was derived by a sample taken over 201 minutes which represents the employee's 8-hour workday, on or about 10/22/14.

The noise training did not include the effects of noise and the advantages and disadvantages of the difference type of hearing protection, on or about 10/22/14. b) Kubota Manufacturing of America Corporation, Gainesville, GA - An employee working at the deck line, station 7, was exposed to a projected noise level of 91.3 dBA. The noise training did not include the effects of noise and the advantages and disadvantages of the difference type of hearing protection, on or about 10/22/14. c) Kubota Manufacturing of America Corporation, Gainesville, GA - An employee working at the deck line, station 9 was exposed to noise levels at a TWA of 90.1 dBA.
