29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested, or required to use the respirator in the workplace:    a)  Imageone Industries - Steel Shop - On or about October 16, 2013, Welders wearing half-face elastomeric respirators to protect them from hazards associated with welding fume, were not provided a medical evaluation by the employer to determine the employee's ability to use a respirator, before the employee began to use the respirator in the workplace.  Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.  Imageone Industries, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 936309, citation number 1, item number 2-b, and was affirmed as a final order on January 14, 2014, with respect to a workplace located at 677 Dunksferry Rd, Bensalem, PA 19020.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. ABATEMENT NOTE: By this date the employer must either correct the alleged violation or implement a Fire Safety Policy, as outlined in 29 CFR 1910.38(a) and (b) which included the evacuation requirement of 29 CFR 1910.157(b).
