OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from crushing hazards:    (a) Facility, 11790 Perry Highway, Wexford, PA:  On or about 10/14/14 the Rotary straddle lift in Bay 1 had inoperable locks and the lift was not taken out of service in accordance with the manufacturer's recommendations.  The locks would stick in place and employees used tools such as screwdrivers to attempt to free the lock to lower the lift.  This condition exposed employees to potential crushing hazards from the falling locking arm and lift.    (b) Facility, 11790 Perry Highway, Wexford, PA:  On or about 10/14/14 the Rotary straddle lift in Bay 3 had hydraulic cylinders that leaked when the lift was taken to its highest point and the lift was not taken out of service in accordance with the manufacturer's recommendations.
