OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from crushing hazards: (a) Facility, Bay 3, 221 Ohio River Boulevard, Sewickley, PA: On or about 10/14/14 a locking arm on the Rotary straddle lift was broken and the lift was not taken out of service in accordance with the manufacturer's recommendations. This condition exposes employees to potential crushing hazards should the hydraulics on the lift fail and the locking arm, being broken, was not capable of holding the lift in place.
