29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) Antonio Sanchez @ 3426 Ulster Street in Denver, CO 80238: On or before October 19, 2014 the employer did not ensure employees conducting roofing operations on a steep roof were protected from fall hazards in that they were improperly wearing their personal fall arrest systems.  Employees were observed using the side and/or front attachment points of their body harnesses.  This condition exposed employees to physical trauma in the event of a fall.
