Employees were exposed to amputation hazards while pushing the cutting head back to the starting position, on or about October 21, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

The electrical components of the junction boxes (metallic outlet box, metal connectors, flexible cord) were not used in accordance with each component listing and exposed the employees to a contact with electrical current hazard, as observed on or about October 21, 2014. b) Drapery, 111 Woodbine Lane, Danville, PA - Employees used in-house made junction boxes with plug-in jacks daily to provide electrical power from a trolley busway to equipment. The electrical components of the junction boxes (metallic outlet box, metal connectors, flexible cord) were not used in accordance with each component listing and exposed the employees to a contact with electrical current hazard, as observed on or about October 21, 2014. c) Bedding, 111 Woodbine Lane, Danville, PA - Employees used in-house made junction boxes with plug-in jacks daily to provide electrical power from a trolley busway to equipment. The electrical components of the junction boxes (metallic outlet box, metal connectors, flexible cord) were not used in accordance with each component listing and exposed the employees to a contact with electrical current hazard, as observed on or about October 21, 2014. d) Inspection station directly northeast of the Utica Mill Specialty Machinery Slitter, Bedding, 111 Woodbine Lane, Danville, PA - Employees operated a fluorescent light daily by plugging/unplugging a flexible cord (w/attachment plug) that provided electrical power to the light by a connection within a metallic outlet box. The electrical components (flexible cord, attachment plug, metallic outlet box) were not used in accordance with each component listing and exposed the employees to a contact with electrical current hazard, as observed on or about October 21, 2014.
